How to apply step by step
Step 1
Call NDIS
on 1800 800 110 or ask your GP for an Early Childhood partner referral (children under 9)
Step 2
Gather evidence
Paediatrician diagnosis, school reports, allied health assessments
Step 3
Complete Forms
Complete the Access Request Form. Your early childhood partner will help
Step 4
NDIS Meeting
Attend an NDIS planning meeting to set goals and create your child's plan
Step 5
Plan approved
